Whenever the photographer takes picture of people he has to get a permission of the model that she/he allows to use their images in some kinds of media institutions. A model should sight a special form with the permission. If the model is not 18 years old he/she must ask parent (guardian) to sigh the form if they allows their child to appear in the photoshoot. This contract is the official document which helps to avoid any illegal possibilities in order to use the model images. the model whoo sighed this contract gives her permission to use her professional images in the mass media in order to promote CD with her music.

Name of the photographer/s
Model/s name
Product
Date/s of the photoshoot
Where the images is going to be used
Signature of the model or her official representer.
